Below are the Athletic forms required for participation in Athletics throughout the school year and summer for camps. Some of these forms are completed in your annual enrollment packet and have already been submitted once your student is enrolled. BG Physical Form, must be signed by Physician
Preparticipation Physical Evaluation/Medical History, must be signed by Parent/Guardian
Liability Release Form, must be signed by Parent/Guardian
